Clinique Maymard

A receptionist is available in the entrance hall (lower ground floor) to guide you and help you with your administrative formalities, every day from 7.00 am to 6.30 pm, except Sundays from 9.00 am. After these hours, please contact the service on the 1st floor.
When you are admitted, you will need to present various documents, depending on the nature of your hospitalisation:
If you are not due to be admitted:
Ask a member of your family to come to the admissions office to complete your administrative file.
Admission formalities will be kept to a minimum, and the file will be completed later by the patient himself at his bedside or by a member of his family or friends at the admissions office.
Your identity will enable you to be recognised both administratively and medically.
If you are admitted following a consultation or appointment, this will be scheduled for the afternoon:
On the day of your admission, announce yourself at reception and a receptionist will take charge of you. You will need to bring the following documents with you:
- Valid identity document
- Family record book for minor patients
- Up-to-date social security card or membership card for another organisation
- Mutual insurance card or agreement to cover costs if you are covered by a complementary health insurance scheme
- European social security card (for European Union nationals) or application for reimbursement (for non-EU nationals), residence permit or passport (for foreign nationals working in France).
